We would be grateful if you could arrive on time for your appointment. When you arrive, please tell the receptionist that you are here so that you can be checked in on the computer, or at East Leake you can check yourself in using the touch screen in the waiting area.

Please let us know as soon as possible if you are unable to attend your appointment as we may be able to offer it to someone else.
Telephone consultations are designed for medication review, follow up appointments or general problems, where no examination is needed.
Please book these in the same way as for other appointments.The doctor will then ring you at a given time and your problem can be discussed.
Please come to the Health Centre/Surgery if at all possible. We are better equipped to help you here. Remember we can see several people at the surgery in the time it takes to do one home visit.
If you need a home visit please telephone before 10.30am as this helps the doctor plan his calls.
